Addison County Home Health and Hospice, Inc. (ACHHH, Home Health) was founded in 1968 by a committed group of Addison county residents to fill an unmet need for community based professional home care services. It was the intent to make a broad spectrum of home care services available to families and individuals of all ages, regardless of extent of illness or injury. Governed by a volunteer board of directors, Addison County Home Health and Hospice, Inc. is a non-profit agency certified by Medicare to provide comprehensive home care services in Addison County, Vermont. The professional services of Addison County Home Health and Hospice, Inc. are skilled nursing, physical therapy, speech therapy, occupational therapy, medical social work, and nutritional consultation. Ancillary support services include home health aide, personal care attendant, and homemaker.

It is the mission of Elderly Services to provide creative, high quality programs to help elders live safe and satisfying lives in their own homes and communities.
There is no obligation for calling and inquiring about our services. A social worker will arrange an initial free consultation at our site. Home visits can also be arranged. We work with families to make appropriate, affordable financial arrangements for eldercare.
